I ask something along the lines of, "are you planning on having kids/more kids?" That way if they are pregnant they are happy I asked and if not I didn't say anything offensive.

YES! I really almost stuck my foot in it when I met a friend of a friend. She is a skinny little girl who looks 7 months pregnant. It seemed like a no brainer but I remembered the rule. So later when we were talking about our kids she said she had a 4 year old and a one year old and wasn't having anymore. WHEW
